Amikacin/dexlansoprazole

Reactions weekly, 2021

Abstract

Laryngitis: case report A 61-year-old woman developed laryngitis during treatment with amikacin and dexlansoprazole [dosages and routes not stated]. The woman, who had rheumatoid arthritis and mild chronic oesophagitis on dexlansoprazole, presented to the pulmonologist with several weeks of fatigue and decreased exercise tolerance. She was diagnosed with nodular Mycobacterium avium complex (MAC) pulmonary disease without evidence of coinfection or antecedent viral upper respiratory infection [aetiology not stated]. Subsequently, amikacin [amikacin liposome] inhalation suspension (ALIS) once daily was initiated. After five treatments, she experienced hoarseness of voice and difficulty speaking, and after nine treatments, she discontinued ALIS because her symptoms progressed. The voice change severely impacted her daily life, and she was referred to the laryngologist for evaluation. Physical examination revealed a severely raspy, rough, and strained voice with impaired projection, graded 80/100 on the validated Consensus Auditory-Perceptual Evaluation of Voice scale. Laryngovideostroboscopy (LVS) revealed hyperemia of the true vocal folds (VFs) with sessile, plaque-like lesions suggestive of mucosal sloughing or ulceration with fibrinous exudate and absent of VF vibration. A diagnosis of laryngitis and dysphonia was thus made, and amikacin and dexlansoprazole were considered to be the contributing factors [durations of treatments to reactions onsets not stated]. The woman was…
