750 ‘BARI-MED’ AUDIT a Multi-Disciplinary Team Audit Streamlining Medication Prescription on Discharge for Bariatric Patients
Abstract
Abstract Introduction Patients undergoing bariatric surgical procedures must go home post operatively with 6 weeks of liquid/crushed medications as well as a liquid diet as per British Obesity & Metabolic Surgery Society (BOMSS) guidelines. There were inconsistencies in the medications, formulations and durations prescribed on discharge for patients undergoing bariatric procedures. This closed loop audit aimed to improve prescription accuracy of discharge medications dispensed for patients having bariatric procedures at a tertiary centre. Method A retrospective first cycle was performed from July 2023- December 2023. A digital prescription tab/checklist was then created by working with the MDT (Specialist pharmacist, bariatric CNS) based on patient feedback and financial factors. A prospective second cycle was performed from April 2024 – May 2024 after educating junior doctors, ACPs, trainee pharmacists about bariatric surgery guidelines and about the digital new prescription tab. Results Accuracy increased in all medications prescribed on discharge from cycle one (n=35 patients) to cycle two (n=12 patients) after . Paracetamol liquid prescription improved from 49% (cycle 1) to 58% (cycle 2), codeine phosphate tablet from 11% (cycle 1) to 42% (cycle 2), senna liquid from 11% (cycle 1) to 58% (cycle 2) and ondansetron liquid from 20% (cycle 1) to 58% (cycle 2) and lansoprazole dispersible tablet from 97% (cycle 1) to 100% (cycle 2). Enoxaparin prescription was consistently…
