Amantadine combined with extended-release levodopa for the management of motor symptoms in dementia with lewy bodies: a case report
Abstract
This study was conducted within the framework of the initiative research project “The Role of Sleep Disorders and Circadian Rhythm Disturbances in the Diagnosis, Prognosis, Rehabilitation, and Comprehensive Treatment of Mental Disorders and Diseases of the Nervous System” (State registration number 0124U000094; 2024-2028). Introduction. Lewy body dementia is a synucleinopathy belonging to the group of atypical parkinsonian disorders and is characterized by parkinsonism combined with cognitive fluctuations and psychotic episodes. The treatment of this condition remains challenging due to the limited response to dopaminergic therapy and the potential for exacerbation of hallucinations. Therefore, the aim of this case report was to highlight the management of dementia with Lewy bodies by addressing both motor symptoms and cognitive decline to maintain quality of life and preserve self-care abilities. Case report. A 75-year-old male patient presented after a fall with acute pain syndrome, severe muscle rigidity, and inability to move independently. Neurological examination revealed signs of parkinsonism and cerebellar dysfunction, while medical history obtained from the patient’s wife indicated recurrent visual hallucinations, REM sleep behavior disorder, and autonomic dysfunction. During the differential diagnostic process, several modifications of the treatment regimen were required with careful consideration of adverse effects and dose efficacy. The patient was prescribed…
How strong is the evidence?
Evidence Hub reads this as a case report, done in people. Its evidence level is 5 of 5 (Very low · case / narrative / animal / in vitro / in silico). Case reports, narrative reviews, and animal, cell or computer-model studies give the weakest evidence for human health.
