Empagliflozin in patients with myocardial infarction: an updated meta-analysis
Abstract
Empagliflozin is one of the sodium-glucose cotransporter 2 (SGLT2) inhibitors that is used to treat type 2 diabetes and has recently been studied for its potential benefits in patients with myocardial infarction.
We performed this meta-analysis to update the present evidence about the safety and efficacy of empagliflozin in patients with myocardial infarction.
We searched PubMed, Scopus, Web of Science, and Cochrane Central Register of Controlled Trials for relevant randomized controlled trials (RCTs) from inception until February 2023. Records were screened for eligible studies, and all relevant outcomes were pooled as mean difference (MD) or risk ratios (RR) with the corresponding 95% confidence intervals (CI) in the meta-analysis models using Review Manager (RevMan version 5.4).
Pooling data from 9 RCTs (7300 patients) showed that empagliflozin was superior to placebo in terms of cardiovascular death (RR = 0.57, 95% CI [0.46, 0.71], P < 0.00001), hospitalization due to heart failure (RR = 0.62, 95% CI [0.48, 0.79], P =0.0001), and systolic blood pressure (SBP) (MD = - 8.59, 95% CI [-13.26, -3.93], P = 0.0003). The overall effect did not favor either of the two groups in terms of body weight (MD = - 1.03, 95% CI [-4.05, 1.98], P =0.5), diastolic blood pressure (DBP) (MD = - 1.48, 95% CI [-4.36, 1.41], P =0.3), eGFR (MD = 2.19, 95% CI [-0.10, 4.48], P =0.06), HbA1c (MD = - 0.10, 95% CI [-0.41, 0.22], P =0.5), NT-proBNP (MD = - 115.9, 95% CI [-271.5, 39.6], P =0.1), and…
How strong is the evidence?
Evidence Hub reads this as a meta-analysis, done in people. Its evidence level is 1 of 5 (Highest · pooled evidence). Pooled evidence from several studies sits at the top of the evidence pyramid. Reported size: 7,300 participants or records.
