Hidradenitis suppurativa and Ehlers-Danlos syndrome: A case-control study
Abstract
To the Editor: Recent findings of hidradenitis suppurativa (HS) lesions colocalizing with striae distensae have led to the proposal that disruption of the mechanical characteristics of the dermis may result in decreased support to the pilosebaceous unit, thereby predisposing to the development of HS lesions when superimposed upon a proinflammatory environment. This hypothesis would be supported by an association of HS with collagenrelated disease. Here, we demonstrate a significant association between HS and Ehlers-Danlos Syndrome (EDS). Using cross-sectional data from Clinformatics Data Mart Database (https://doi.org/10.57761/ a5hg-b442), we identified 13,667 patients with at least 3-year continuous enrollment from January 2013 to June 2019 with a minimum of 2 International Classification of Diseases Clinical Modification, Ninth or Tenth Revision codes (705.83 or L73.2, respectively) for HS filed on different dates within that time period, in addition to a 10:1 age-, sex-, race-, and enrollment-matched non-HS cohort. The prevalence of EDS in patients with and without HS is displayed in Table I. We performed conditional multivariate logistic regressions and observed a 1.65-fold risk of EDS in patients with HS compared with that in patients without HS (95% CI, 1.02-2.69; P 1⁄4 .042) after adjusting for obesity and tobacco use. This association was significant in the age group of $20 years (odds ratio [OR], 2.17; 95% CI, 1.25-3.76; P 1⁄4 .006) but not in the age group of\20…
How strong is the evidence?
Evidence Hub reads this as a case-control, done in people. Its evidence level is 4 of 5 (Low · observational). Observational designs can show an association but not prove cause. Reported size: 13,667 participants or records.
