PS11: Psychiatric associations in bullous pemphigoid: review and case report
Abstract
PS11 Psychiatric associations in bullous pemphigoid: review and case report Sarah Dyson, Priya Patel and George Millington Norfolk and Norwich University Hospital, Norwich, UK A 59-year-old woman with known schizoaffective disorder presented with an itchy blistering generalized rash. Other medical history includes agoraphobia, anxiety, hypothyroidism, asthma, diabetes (type 2, treated with empaglifozin) and hypertension. Her medications included adcal-D3, alendronate, chlorphenamine, citalopram, lansoprazole, lisinopril, paliperidone, levothyroxine and lercanidipine. Serology showed antipemphigoid antibodies. Histology showed subepidermal blistering with perivascular inflammatory infiltrate and occasional eosinophils. Direct immunofluorescence showed the characteristic findings of linear IgG and C3 at the basement membrane. Empagliflozin was stopped, as well as paliperidone. This did not stop the blisters forming. She was treated with Dermovate ointment under paste bandaging to the trunk and limbs and Eumovate ointment for the face, alongside doxycycline 100 mg and prednisolone 40 mg for a week, reducing by 5 mg per week over 8 weeks. However, new blisters continued developing, so dapsone 50 mg was started, resulting in significant improvement. A case of bullous pemphigoid (BP) in a patient with schizoaffective disorder was seen. Increasingly, neurological comorbidities, including dementia, Parkinson disease, cerebrovascular disorders, epilepsy and multiple sclerosis…
How strong is the evidence?
Evidence Hub reads this as a case report, done in people. Its evidence level is 5 of 5 (Very low · case / narrative / animal / in vitro / in silico). Case reports, narrative reviews, and animal, cell or computer-model studies give the weakest evidence for human health.
