Case of onychomycosis due to Candida tropicalis successfully treated using fosravuconazole
Abstract
He had been treated with various kinds of medicines including fentanyl, midazolam, albumin and omeprazole. Because of the possibility that the skin eruption could be caused by drugs, omeprazole which had been administrated from day 9 was changed to an alternative agent. Skin lesions gradually improved 7 days after the treatment with fexofenadine hydrochloride and topical corticosteroids. We confirmed that PCR analysis for SARS-CoV-2 was negative on day 47. In a recent case study, 20.4% of the confirmed COVID-19 patients had developed cutaneous manifestations, and the most common cutaneous presentation was erythematous rash (77.8%), which was associated with a severer course of the disease. Because there is still no effective medicine for COVID-19 pneumonia, COVID-19-infected patients need to be treated with many kinds of medicine. This leads to an increased risk of drug eruptions and makes it difficult to determine whether skin lesions are associated with COVID-19 infection or drugs. In the present case, PCR for SARS-CoV-2 virus was positive and changed to negative just after remission in erythematous lesions, which strongly indicated that skin lesions were associated with COVID-19 infection. Furthermore, a recent report suggested that the viral particles present in the cutaneous blood vessels in patients with COVID-19 infection could lead to immune response at keratinocytes and subsequent induction of a variety of cutaneous manifestations. Erythematous lesions were observed…
