Re: Efficacy of Tadalafil Therapy and Changes in Oxidative Stress Levels in Male Patients with Lower Urinary Tract Symptoms and Overactive Bladder
Abstract
available at http://www.ncbi.nlm.nih.gov/pubmed/31407871 Editorial Comment: This prospective study included 53 men who met the criteria for overactive bladder (OAB), ie 2 or more points for question 3 in the overactive bladder symptom score assessment and 3 or more points for the total score. Patients received 5 mg tadalafil once daily, and their symptoms were assessed before and after treatment. Oxidative stress was evaluated by determining urinary levels of 8-hydroxy-2’-deoxyguanosine (8-OHdG) with an adjustment for urinary creatinine concentration. The total and individual indices of the OAB symptom score showed significant improvement, and the total storage and voiding symptoms that contribute to the International Prostate Symptom Score were also improved. In patients who showed OAB improvement and did not meet the criteria for OAB after treatment the oxidative stress marker was significantly decreased. This study adds to the growing amount of evidence that oxidative stress is certainly associated with some types of lower urinary tract symptoms, and improvement is associated with a reduction in markers attributed to oxidative stress. Whether this can be construed as support for cause and effect and therefore the development of treatments to decrease oxidative stress as a method for managing certain types of lower urinary tract symptoms is an intriguing possibility that remains to be confirmed. How strong is the evidence?
Evidence Hub reads this as a randomized trial, done in people. Its evidence level is 2 of 5 (High · randomized trial). Randomized trials of a reasonable size are the strongest single-study design. Reported size: 53 participants or records.
