Efficacy of Tadalafil 5mg Versus Its Combination with Pelvic Floor Muscles Exercises in Treatment of Men with Monosymptomatic Post-Micturition Dribble: Prospective, Randomized, Comparative Study
Abstract
Background: Post-micturition dribbling (PMD) is common in men with lower urinary tract symptoms (LUTS), affecting over 50%. It is linked to erectile dysfunction (ED) and results from residual urine in the urethra. Treatments include pelvic floor exercises and alpha-adrenergic antagonists for bladder obstruction, with side effects like dizziness. PDE-5 inhibitors like tadalafil may alleviate PMD by enhancing urinary flow and muscle tone. Objectives: To compare the efficacy of tadalafil 5mg alone versus its combination with pelvic floor muscles exercises to evaluate outcome on PMD in treatment of post-micturition dribble in men without lower urinary tract symptoms. Patients and methods: This study upon men aged 20-45 with post-micturition dribble but no other urinary symptoms. 70 participants divided into two groups: one received tadalafil 5 mg daily, the other tadalafil plus Kegel exercises over three months. Evaluation included Hallym Post-Micturition Dribble Questionnaire and clinical assessments like vital signs, urological exams, and ultrasound. Results: Both groups had similar age and residency distribution. Combination therapy showed significant PMD improvement: reduced HPMDQ scores at 2 (4.97 vs. 6.2, p=0.0137*) to 10 weeks (0.09 vs. 1.37, p=0.0051*), fewer dribbling instances (2.63 vs. 2.97, p=0.0377* to 0.11 vs. 0.46, p=0.0798), and decreased dribble volume (7.91 vs. 9.74, p=0.0214* to 0.34 vs. 1.23, p=0.1178) versus tadalafil alone.
