Background: The impact of vitamin D supplementation on the incidence of fracture in healthy elderly individuals remains uncertain. Objectives: We performed a systematic review and meta-analysis of studies investigating the incidence of fracture in healthy elderly patients with supplementation of vitamin D at doses exceeding 800 IU per day in comparison to placebo. Methods: We systematically searched PubMed, Embase and Cochrane Central Register of Controlled Trials in November 2023 with the main following search terms: ‘Vitamin D’, ‘supplementation’, ‘elderly’, ‘fracture’ and ‘randomized trial’. The initial search revealed 2,160 articles that were screened and assessed according to inclusion and exclusion criteria. Risk ratio (RR) and Hazard ratio (HR) with 95% confidence intervals were used to compare treatment effects for categorical endpoints. Heterogeneity was evaluated with Cochran Q test and I2 statistics; p values inferior to 0.10 and I2>25% were considered significant for heterogeneity. Results: A total of seven randomized controlled trials with 71,899 patients were included, of whom 36,822 (51.2%) were women. A total of 25,916 (78.2%) participants received a daily dose of vitamin D higher than 1,500 UI. The incidence of total fractures was not significantly different between participants who received vitamin D or placebo (RR 1.01; 95% CI…

How strong is the evidence?

Evidence Hub reads this as a meta-analysis, done in people. Its evidence level is 1 of 5 (Highest · pooled evidence). Pooled evidence from several studies sits at the top of the evidence pyramid. Reported size: 71,899 participants or records.

How the five levels of evidence work. The level is assigned automatically from the abstract and can be wrong, so check the paper.
