Delayed Slipped Capital Femoral Epiphysis Following Surgical Fixation of a Femoral Neck Fracture in a Child: A Rare Case Report
Abstract
Introduction: Slipped capital femoral epiphysis (SCFE) occurring after surgical treatment of femoral neck fractures in children is exceedingly rare. Awareness of this complication is essential because delayed diagnosis may result in progressive deformity and significant morbidity.
Case report: We report the case of a 9-year-old female child who sustained a right-sided femoral neck fracture (Delbet Type III) and a left subtrochanteric femoral fracture following a fall from height. She underwent closed reduction and internal fixation of the femoral neck fracture with cannulated cancellous screws and open reduction with plating for the subtrochanteric fracture. Although fracture union was achieved, residual coxa vara deformity was noted. Fifteen months postoperatively, she developed hip pain and limping. Radiographs revealed SCFE with retained implants in situ. Endocrine evaluation was normal. Revision surgery was advised; however, the patient was lost to follow-up.
Conclusion: SCFE developing after femoral neck fracture fixation in children is a rare but important complication. Residual deformity, altered proximal femoral biomechanics, and possible physeal stress may predispose to slip. Anatomical reduction, stable fixation, and careful post-operative management are crucial to minimize this risk.
How strong is the evidence?
Evidence Hub reads this as a case report, done in people. Its evidence level is 5 of 5 (Very low · case / narrative / animal / in vitro / in silico). Case reports, narrative reviews, and animal, cell or computer-model studies give the weakest evidence for human health.
