Abstract

QTc-interval prolongation and cardiac arrest following off-label therapy: 3 case reports A case series described 3 patients (2 women and 1 man) aged 50–75 years, who were hospitalised in Iran between March–April 2020 due to Coronavirus disease-19 (COVID-19) infection, and developed QTc-interval-prolongation following offlabel therapy with hydroxychloroquine, as well as cardiac arrest following off-label therapy with levofloxacin, hydroxychloroquine, lopinavir/ritonavir, azithromycin or oseltamivir [dosages and routes not stated]. Case 1: A 50-year-old woman developed QTc-interval prolongation following off-label treatment with hydroxychloroquine and cardiac arrest following off-label treatment with levofloxacin, hydroxychloroquine and lopinavir/ritonavir. The woman was admitted to the hospital with complaints of chills, fever and dry cough. Based on these symptoms, she was diagnosed with COVID-19 infection. On admission, her BP was 130/80mm Hg, body temperature was 37.2°C, pulse rate was 80 /minute and O2 saturation on room air was 95%. Due to the pulmonary involvement (COVID-19 pneumonia), she started receiving off-label treatment with levofloxacin, lopinavir/ritonavir, hydroxychloroquine and vancomycin, concomitantly with heparin for prophylaxis of deep venous thrombosis at standard doses. Baseline ECG was found to be normal. However, she was found to have prolonged QT and QTc intervals at 380ms and 480ms, respectively secondary to hydroxychloroquine therapy. During…

How strong is the evidence?

Evidence Hub reads this as a case series, done in people. Its evidence level is 5 of 5 (Very low · case / narrative / animal / in vitro / in silico). Case reports, narrative reviews, and animal, cell or computer-model studies give the weakest evidence for human health.

How the five levels of evidence work. The level is assigned automatically from the abstract and can be wrong, so check the paper.
