Virtual Trials in Nuclear Medicine Using Digital Patient Twins and Scanners
Abstract
Purpose: The problem of quantitative SPECT based on the assessment of the standardized uptake value (SUV) remains unresolved: the accuracy of this assessment is far from always reliable and depends on the technical protocol and software settings. Clinical studies are significantly limited in addressing this problem due to the inability to assess the accuracy of the obtained images by comparing them with the true distribution of the radiopharmaceutical (RP) in the patient's organs and tissues. An effective approach to solving this problem is mathematical simulation modeling using digital twins of patients and physical standardized phantoms, as well as SPECT imaging devices. A digital twin with a given RP distribution serves as a standard for assessing the accuracy of reconstructed images. The aim of this study, based on the simulation modeling method, is to investigate the effect of the settings of the widely used OSEM (Ordered Subset Expectation Maximization) reconstruction algorithm on the accuracy of quantitative SUV assessments.
Material and methods: The study was conducted using the software package, "Virtual Platform for Simulation Testing of the SPECT/CT Method". Virtual tests were performed using a digital twin of a standardized NEMA IEC phantom and a digital twin of the patient. A clinical case of SPECT/CT examination of a patient with metastatic bone lesions was simulated. The accuracy of quantitative assessment of lesions in reconstructed images was determined…
