Abstract

We read the article by Dr Shahat et al. [1] with great interest. In their randomized, placebo-controlled clinical trial, they investigated the effect of preoperative tamsulosin on the outcomes of semirigid ureteroscopy for lower ureteric stones. A total of 80 patients were enrolled and equally randomized into tam-sulosin and placebo groups. Baseline characteristics, including age, sex, stone size, radiopacity, and degree of obstruction, were comparable between the groups. Operative time was significantly shorter in the tamsulosin group ( p = 0.001). The need for ureteral dilation (20% vs. 80%, p < 0.001) and postoperative stenting ( p = 0.001) was also significantly lower among the tamsulosin group. Although the stone-free rate was higher in the study group, the overall complication rates did not differ significantly between groups. Therefore, the authors concluded that preoperative administration of tamsulosin improves the surgical outcomes of ureteroscopy for lower ureteric stones by reducing operative time, the need for dilation, and stent placement, without increasing complication rates. Similarly, Dr Reda et al. [2] evaluated the efficacy of tadala-fil alone versus a combination of tadalafil and tamsulosin as medical expulsive therapy (MET) for lower ureteric stones measuring ≤10 mm. Patients were randomized into 3 equal groups: Group

How strong is the evidence?

Evidence Hub reads this as a randomized trial, done in people. Its evidence level is 2 of 5 (High · randomized trial). Randomized trials of a reasonable size are the strongest single-study design. Reported size: 80 participants or records.

How the five levels of evidence work. The level is assigned automatically from the abstract and can be wrong, so check the paper.
