Abstract

Introduction: The rapid proliferation of electronic cigarettes (e-cigarettes) has outpaced scientific understanding of their long-term health effects. A critical public health question is whether e-cigarette use independently increases the risk of early-onset Chronic Obstructive Pulmonary Disease (COPD), particularly in younger populations and never-smokers, a question complicated by high rates of dual use with combustible tobacco. Methods: This systematic review synthesized evidence from 80 RCT, etc. Data were extracted on study design, e-cigarette exposure definition, COPD diagnostic criteria (spirometry, ICD codes), control for combustible tobacco use (including pack-years), and risk estimates (OR, RR, aHR). A focused analysis was performed on studies restricting to never-smokers and younger adults (ages 18-50) to assess independent risk. Results: Significant positive associations between e-cigarette use and COPD/respiratory disease were found. Four major meta-analyses reported pooled ORs for current e-cigarette users versus non-users ranging from 1.48 (95% CI:1.36-1.61) to 1.84 (95% CI:1.51-2.23) (1,2,3,41). Crucially, in never-smokers, exclusive e-cigarette use was significantly associated with COPD (aHR 2.29,95%CI:1.42-3.71)(8) and a pooled OR of 1.44 (95%CI:1.42-1.46)(7). A dose-duration response was identified (aHR 1.28 per year of use, 95%CI:1.16-1.40)(9). Conversely, studies adjusting for combustible cigarette pack-years found no significant independent risk (aHR…

How strong is the evidence?

Evidence Hub reads this as a systematic review, done in people. Its evidence level is 1 of 5 (Highest · pooled evidence). Pooled evidence from several studies sits at the top of the evidence pyramid.

How the five levels of evidence work. The level is assigned automatically from the abstract and can be wrong, so check the paper.
