Comparing the Effects of Agnugol and Metformin on Oligomenorrhea in Patients with Polycystic Ovary Syndrome: A Randomized Clinical Trial
Abstract
Polycystic Ovarian Syndrome (PCOS) is the most common endocrinopathy and cause of infertility in women [1,2].Its pathophysiology is complex and obesity and insulin play a major role in its pathophysiology.Anovulation is present in 60% of women with PCOS, and oligomenorrhea or secondary amenorrhea, diagnosed by laboratory and clinical evidence of androgen excess [3].Hormone imbalances such as anovulation, and androgen excess disorders such as PCOS, adrenal hyperplasia, hyper prolactinemia, and thyroid function disorders are the most important causes of oligomenorrhea [4,5], seen when menstruation intervals are more than 35 days [6], or the total numbers of cycles are 5-7 a year [7].Statistics report the prevalence of oligomenorrhea10-15% and in infertile women, 10-20%.In Iran, 14-26% prevalence has been reported [8].Pathologies caused by oligomenorrhea such as infertility, low fertility, malignancies, cardiovascular diseases, diabetes mellitus, hirsutism and acne vulgaris result in problems regarding women's general health and quality of life. Therefore, timely diagnosis and treatment of these disorders are of utmost importance [9,10].Anovulation is one of the most important causes of oligomenorrhea in PCOS patients.Metformin is considered the first line treatment for ovulation disorders and infertility [2].Serum insulin levels are high in these patients; Metformin decreases the insulin levels, LH, and androgen in the ovaries via gluconeogenesis inhibition in liver, glucose…
