Editorial: it is not a wide‐open field for incretins ‐ collateral benefits favour the use of metformin in advanced non‐alcoholic steatohepatitis
Abstract
© 2019 John Wiley & Sons Ltd prolonged hypophosphataemia after only one infusion with ferric carboxymaltose. Symptoms of hypophosphataemia are non‐specific and highly dependent on cause, duration and severity. Studies from other patient populations than IBD indicate a relationship between hy‐ pophosphataemia and cardiac function.5,6 Case series have clearly demonstrated that sustained hypophosphataemia can result in os‐ teomalacia with pseudofractures.7 IBD patients and, especially, pa‐ tients with Crohn's disease have a high incidence of metabolic bone disease,8,9 and hypophosphataemia could be an additional risk factor for deterioration of bone health in these patients. Although there are uncertainties with regard to the clinical impact of hypophospha‐ taemia, it is our conviction that it should be avoided. Intravenous iron therapy is very efficacious but, as demon‐ strated in our study, most IBD patients treated with intravenous iron need high doses and/or repetitive infusions. This further under‐ lines the importance of addressing the risk for hypophosphataemia in this patient group. We fully agree that measuring serum phos‐ phate level is mandatory when monitoring patients receiving ferric carboxymaltose.
How strong is the evidence?
Evidence Hub reads this as a case series, done in people. Its evidence level is 5 of 5 (Very low · case / narrative / animal / in vitro / in silico). Case reports, narrative reviews, and animal, cell or computer-model studies give the weakest evidence for human health. Reported size: 7 participants or records.
