Retrospective Study of the Efficacy and Safety of Fosravuconazole Switching Therapy in Patients with Onychomycosis Resistant to Topical Antifungal Treatment
Abstract
実臨床下において爪外用抗真菌薬（以下，外用薬）からホスラブコナゾールL-リシンエタノール付加物（ネイリン®カプセル，以下，F-RVCZ）に切り替えられた爪白癬患者を対象に，F-RVCZの有効性と安全性に関する，単機関，探索的，後ろ向き観察研究を実施した． 前治療薬である外用薬の平均投与期間は24.3ヵ月で，いずれの症例も効果不十分でF-RVCZに切り替えられ，治療開始時の平均混濁比は7.8と重度の症例が多かったが，F-RVCZの有効性評価対象例では48週時の臨床的治癒率は62.5％であったことから，外用薬からの切り替え時にもF-RVCZは高い有効性を示すことが明らかとなった．副作用が13例（25.0％）に認められ，うち3例が中止に至ったが，いずれも既知の症状で軽快・回復が確認された．外用薬で効果不十分の場合，相互作用や安全性への懸念が比較的少なく，12週間の短期投与で高い治療効果が期待できるF-RVCZは切り替え先として有用な選択肢の一つとなり得る．
