Efficacy and safeness of tralokinumab in patients with atopic dermatitis who developed conjunctivitis under dupilumab: A case series
Abstract
Dear Editor, Dupilumab is the current gold standard therapy for moderate– severe AD. It is a human monoclonal IGg4 antibody which binds the common alfa chain of the interleukin (IL) 4 and 13 receptors, two key mediators in AD pathogenesis.1 Nevertheless, there are still unmet needs for some patients with severe AD. In fact, dupilumab seems to be less effective in patients with face eczema and inductor of conjunctivitis which impacts negatively on quality of life.2– 5 Tralokinumab is a new human monoclonal IGg4 antibody which specifically binds the IL13; unlike dupilumab there is no interaction with IL4 signalling pathway.6 We have retrospectively collected data of different centers about 14 AD patients treated with dupilumab who developed a moderate to severe form of conjunctivitis and then were switched to tralokinumab with consequent resolution both of ocular and cutaneous disease (Table 1). Dupilumab 300 mg (subcutaneous injections— two injections at the same time and then one every 2 weeks) was prescribed. After an initial eczema improvement, all patients developed a conjunctivitis during the therapy. Seven patients had a previous story of it. The onset occurred after an average of 9 weeks. Six patients presented a severe conjunctivitis. All patients were repeatedly examined by ophthalmologists. Topical steroids, emollients and tacrolimus were prescribed to everyone for eyelids, eight patients assumed steroids eye drops, and cyclosporine eye drops were applied by four…
How strong is the evidence?
Evidence Hub reads this as a case series, done in people. Its evidence level is 5 of 5 (Very low · case / narrative / animal / in vitro / in silico). Case reports, narrative reviews, and animal, cell or computer-model studies give the weakest evidence for human health. Reported size: 14 participants or records.
