Abstract

Abstract Introduction Peyronie’s disease (PD) results from fibrosis of the penile tunica albuginea and can present with pain, penile deformity, and erectile dysfunction. Available therapeutic strategies are limited to surgery or intralesional injections. Our previous in vitro and in vivo research has suggested that combining phosphodiesterase type 5 inhibitors (PDE5i; e.g., sildenafil, tadalafil) with selective oestrogen receptor modulators (SERMs; e.g., tamoxifen, raloxifene) yields synergistic anti-fibrotic activity. Objective To evaluate the clinical effect of tamoxifen with a PDE5i compared to standard management in men presenting with acute PD. Methods Penile pain and curvature were measured at baseline and after three months of treatment. A cohort of 133 men (average disease duration 7.1 months) attending a private hospital received tamoxifen (20 mg twice daily) together with sildenafil (50 mg) or tadalafil (5 mg) daily. A comparator group of 26 men (mean duration 7.8 months) at a non-private hospital received routine care, consisting of either observation or vitamin E. Results Patients treated with the drug combination exhibited a significant reduction in penile curvature, while no such improvement was seen in the standard care group. Pain scores decreased in both cohorts, but the improvement was more pronounced with tamoxifen plus PDE5i therapy. Conclusions These findings provide the first clinical evidence that concurrent use of tamoxifen and a PDE5i may stabilise…

How strong is the evidence?

Evidence Hub reads this as a cohort study, done in people. Its evidence level is 3 of 5 (Moderate · trial / cohort / secondary analysis). Trials without a large randomized design, cohorts and secondary analyses give moderate evidence. Reported size: 133 participants or records.

How the five levels of evidence work. The level is assigned automatically from the abstract and can be wrong, so check the paper.
