Abstract

Abstract Introduction: Delftia acidovorans and Stenotrophomonas maltophilia are typically non pathogenic, gram negative, aerobic, non fermenting bacilli found in environmental sources like soil and water. While S. maltophilia is more common and recognized for its intrinsic drug resistance, causing challenging infections, D. acidovorans infections are much rarer and are of particular clinical significance in immunocompromised individuals. D. acidovorans infections causing pneumonia and sepsis are often associated with an increased susceptibility to secondary infections from other gram-negative bacteria, especially in patients with solid or hematologic malignancies. Here, we report a rare case of severe D. acidovorans pneumonia complicated by a concurrent S. maltophilia infection in a patient with metastatic squamous cell carcinoma. Case Presentation: A 78 year old female with a history of tobacco abuse, stroke, peripheral artery disease, type II diabetes, and hypertension originally presented with altered mental status and weakness to the emergency department. Computed tomography of the head revealed an acute left middle cerebral artery stroke. Despite mechanical thrombectomy, her condition worsened, and she required intubation for respiratory failure. Her hospital course was complicated and she underwent tracheostomy and percutaneous gastrostomy placement. Further imaging revealed extensive lymphadenopathy, loculated right pleural effusion, and a right lung mass with…

How strong is the evidence?

Evidence Hub reads this as a cohort study, done in people. Its evidence level is 3 of 5 (Moderate · trial / cohort / secondary analysis). Trials without a large randomized design, cohorts and secondary analyses give moderate evidence. Reported size: 59 participants or records.

How the five levels of evidence work. The level is assigned automatically from the abstract and can be wrong, so check the paper.
