Abstract
Objective To compare the appropriateness of levofloxacin prescribing between patients managed with and without a Drug Use Evaluation (DUE) program. Methods A retrospective cohort analytic study was conducted using electronic medical records from Cha-am Hospital. A total of 201 patients were included, with 98 in the pre-DUE group and 103 in the post-DUE group. Prescribing appropriateness was evaluated based on clinical practice guidelines, including Infectious Diseases Society of America (IDSA) guidelines and Thai national guidelines, considering indication, dose, and duration of therapy. Data were analyzed using the chi-square test, Fisher’s exact test, Mann–Whitney U test, and multivariable logistic regression. Results The post-DUE group had a lower mean age (59.23±17.31 vs. 68.20±16.92 years; p=0.001) and a higher proportion of culture-positive patients (61.2% vs. 40.8%; p=0.005). The proportion of appropriate prescribing significantly improved in terms of duration of therapy (100% vs. 93.9%; p=0.037). No statistically significant differences were observed for indication (71.8% vs. 62.2%; p=0.161) or dose (100% vs. 96.9%; p=0.119). In multivariable analysis, the DUE program was not significantly associated with appropriateness of indication (adjusted OR 0.36; 95% CI 0.05–2.41; p=0.290), while culture results were significantly associated (adjusted OR 0.15; 95% CI 0.08–0.31; p<0.001). Conclusion The post-DUE period was associated with improved appropriateness of…
How strong is the evidence?
Evidence Hub reads this as a cohort study, done in people. Its evidence level is 3 of 5 (Moderate · trial / cohort / secondary analysis). Trials without a large randomized design, cohorts and secondary analyses give moderate evidence. Reported size: 201 participants or records.
